• W-2 - Last years W-2 and/or 1099's.
• Tax Returns (if self-employed) - Last 2 yrs.
• 1st Mortgage Statement - Most current statement.
• 1st Mortgage Note - Obtain from the closing package you received when you originally obtained the loan.
• 2nd Mortgage Statement (If applicable) - Most current statement.
• Home Insurance Policy Information - Name of company and phone number.
